Trek Registration and Medical Release Form
The Trek Registration and Medical Release form must be completed, signed, and turned in at the Trek Kickoff Fireside held on Sunday, April 24, 2022 or to your ward youth leaders by that date. Each participant (adult and youth) must complete a registration form.
